Trachoma

This man was referred in for trichiasis. He complained of needing lashes removed from his right lower eyelid every month or so for the last few years.

Whilst I can help, the question is why does a 75 yr old man develop trichiasis?

Evert the lids and find out: dramatic conjunctival scarring, involving all 4 lids.

The trichiasis is the effect of previous conjunctivitis, likely viral. Other things to think of include trachoma and mucus membrane pemphigoid.

I will undertake local radiofrequency ablation of the lashes, and have guaranteed him that he will have recurrence at some stage.

I no longer use or advocate treating such lashes with laser or electrolysis or cryotherapy: these techniques are application, but not effective. No doubt you will have come across many patients having repeated laser treatment or electrolysis with little benefit.
